What you'll do

• Perform patient assessments
• Develop home health care plans
• Provide skilled nursing interventions
• Monitor patient progress
• Educate patients and caregivers
• Coordinate with physicians and therapists
• Document home health visits
• Escalate significant changes in patient condition

What we're looking for

• Current Registered Nurse licence
• Clinical nursing experience
• Strong assessment skills
• Patient education ability
• Excellent documentation skills
• Ability to work independently
• Strong communication
• Home healthcare experience preferred
